               Club:     Club name
      
               E-mail:    your email address

              Address:   your mailing address

         ***Phone:   home AND cell  (in case your chairman needs
                              to reach you;  not for publication)

STUDENT LISTING:  (see Duet rules below)

              List by ALPHABETICAL ORDER,  last name first. 

      Use tab to move cursor to where the following are:

              Previous Consecutive Superiors; 
                              put how many, or if  'new',  put 0

             Previous Accumulated Points; 
                              put how many, or if   'new',  put 0.

             Class:  PP,  P1,  P2,  P3, P4, E1,  etc.  as listed in Bulletin.

             Notes: 
                       put the last level and year that the student played. 
                          (Example:   P4 , '13). 
                      So that student played last year in P4.   
                     If a student has never participated in Festival before,
                                and is 'new',   please put  NEW.

             TO LIST STUDENTS FOR DUETS:

                          List by PAIRS  and list alphabetically. LAST NAME first

                 Skip a line between pairs.

                 If a student is  'assisting' , put  that by their name.
